Make Your Wishes Known—Advance Care Planning Workshop.
Join us in person or via the weblink in your confirmation email.
Planning for unexpected healthcare emergencies is important at any age as a guide for your medical caregivers and a gift to your loved ones. Are there certain conditions when you would want medical treatment stopped? Making healthcare decisions like these in advance helps your loved ones know your wishes and be able to carry them out should the situation arise.
Join us for a free Advance Care Planning workshop on April 16, National Healthcare Decisions Day. The goal of this workshop is to bring awareness to the importance of advance care planning and how to complete your advance directives.
Notaries will be available on site, if you chose to complete your documents in person. Employees will earn 10 points toward their annual wellness incentive by completing advance directives and attesting via the StayWell portal.
Date: Tuesday, April 16
Time: 12 PM – 1 PM
Location: Valley Medical Center, Medical Arts Center Conference Room E/F or YouTube live streaming
